Zucchini Pasta Bar, a restaurant, cafe or canteen at 42a Garden Walk, Metro Centre, Whickham was given the score after assessment on August 10, the Food Standards Agency's website shows.
It means that of Gateshead's 260 restaurants, cafes and canteens with ratings, 196 (75%) have ratings of five and just two have zero ratings.
