Rosa 12, a restaurant, cafe or canteen at 580-580a Durham Road, Low Fell, Gateshead was given the score after assessment on September 14, the Food Standards Agency's website shows.
It means that of Gateshead's 258 restaurants, cafes and canteens with ratings, 197 (76%) have ratings of five and just two have zero ratings.
