Momins, a takeaway at 7 Phoenix Chase, North Shields, Tyne And Wear was given the score after assessment on August 22, the Food Standards Agency's website shows.
It means that of North Tyneside's 182 takeaways with ratings, 101 (55%) have ratings of five and none have zero ratings.
