Wing Chau, a takeaway at 14 Buttermere Road, Cullercoats, Tyne And Wear was given the score after assessment on September 22, the Food Standards Agency's website shows.
It means that of North Tyneside's 185 takeaways with ratings, 103 (56%) have ratings of five and just one has a zero rating.
