Park Lane Cards, a restaurant, cafe or canteen at 136 Fowler Street, South Shields, Tyne & Wear was given the score after assessment on July 14, the Food Standards Agency's website shows.
It means that of South Tyneside's 174 restaurants, cafes and canteens with ratings, 111 (64%) have ratings of five and none have zero ratings.
