Red Star Chop Suey House, a takeaway at 121 Marsden Road, South Shields, Tyne & Wear was given the score after assessment on May 17, the Food Standards Agency's website shows.
It means that of South Tyneside's 147 takeaways with ratings, 65 (44%) have ratings of five and just one has a zero rating.
