The Balti Masters, a takeaway at 28 St James Mall, Hebburn, Tyne & Wear was given the score after assessment on July 11, the Food Standards Agency's website shows.
It means that of South Tyneside's 148 takeaways with ratings, 67 (45%) have ratings of five and just one has a zero rating.
