Noodle Noodles, a takeaway at 112 Ocean Road, South Shields, Tyne & Wear was given the score after assessment on July 14, the Food Standards Agency's website shows.
It means that of South Tyneside's 148 takeaways with ratings, 67 (45%) have ratings of five and just one has a zero rating.
