Mary's Fish & Chips, a takeaway at 10 Seton Walk, South Shields, Tyne & Wear was given the score after assessment on March 7, the Food Standards Agency's website shows.
It means that of South Tyneside's 140 takeaways with ratings, 67 (48%) have ratings of five and none have zero ratings.
